Due to the COVID-19 pandemic, we are not conducting face to face appointments at this time. You may schedule a video or phone appointment in order to receive services, and one of our staff will give you a call at your scheduled appointment time.

Instructions for uploading your documents
Please complete the application and prepare all required documents as listed in below section. Once you are ready, please visit https://pacs.itfrontdesk.com/fileupload/ and login with SSN, name, contact phone and email and click "File Upload" button. Or download and install from below Mobile App link and click "Upload Documents" button.

If you have already scanned documents, use file browser to navigate to the appropriate folder and select the files to upload. If you do not have scanned documents, use "camera" option to take a picture of each page and upload it.

Note: If you are using phone camera, please make sure to lay the document on a flat table and hold the phone camera steady at a distance to cover the entire document and it is well focused.

Please upload the documents atleast 24 hours prior to appointment date.

Instructions for joining video conference
At your appointment time, you will receive a meeting invite via Text and/or Email and/or phone call. You just simply click on the link or copy & paste the URL on to a browser. Or download and install from below Mobile App link and click "Start Video Chat" button.

Enter your meeting ID and last 4 digits of your SSN and click "Start Video Chat" button.

List of Documents Needed

Failure to submit all required documentation will delay your assistance. We require below documents:

  • Social security cards for the entire household.
  • Date of birth for the entire household.
  • Proof of income for the entire household for the month prior; including paystubs, award letter, child support verification, and unemployment benefits.
  • Utility bill, eviction notice or rent statement.

Please note after reviewing your documents we may request additional information to complete your application.

Within minutes of making your appointment, you should receive a confirmation email with a calendar appointment attached to the email. In addition, you will also receive a phone reminder the day before your appointment.
